INTERMEDIATE CARE FACILITY (ICF)
St. Coletta’s Intermediate Care Facility provides comprehensive, 24-hour care for individuals with developmental disabilities who have higher support needs. Our ICF home is staffed around the clock, with a dedicated cook and trained staff who assist residents with daily living needs, personal care and health support. Residents receive individualized, active treatment programs based on their Individual Service Plan (ISP), along with a wide range of services, including occupational, speech, physical and recreational therapies. Staff also accompany residents
to medical appointments and support them in the community by assisting with errands such as banking and shopping, helping each individual maintain health, independence and quality of life.
ADMISSIONS INFORMATION
Our facilities accept referrals for individuals who are at least 18 years of age and meet at least one of the following qualifications:

Developmentally disabled

Intellectual disability that results in a need for active treatment and manifested before the age of 18 years

Condition is severe and chronic; attributable to cerebral palsy or epilepsy

Any condition, other than mental illness, that is found to result in impairment of general intellectual functioning or adaptive behavior

Age of onset prior to 22 years old

Likelihood that the condition will continue indefinitely

Substantial functional limitations in three or more of the major life areas, which are self-care, use of language, learning, mobility, self-direction and capacity for independent living
