INTERMEDIATE CARE FACILITY

St. Coletta’s ICF home provides health care services to high needs individuals with developmental disabilities. We offer active treatment programs, interventions and services to support the achievement of the objective identified in the Individual Service Plan. Services include - but are not limited to - occupational, speech, physical and recreational therapies; psychological; psychiatric; audiological; social work; special education; dietary; and rehabilitation counseling.

ADMISSIONS INFORMATION

Our facilities accept referrals for individuals who are at least 18 years of age and meet at least one of the following qualifications:

  • Developmentally disabled

  • Intellectual disability that results in a need for active treatment and manifested before the age of 18 years

  • Condition is severe and chronic; attributable to cerebral palsy or epilepsy

  • Any condition, other than mental illness, that is found to result in impairment of general intellectual functioning or adaptive behavior

  • Age of onset prior to 22 years old

  • Likelihood that the condition will continue indefinitely

  • Substantial functional limitations in three or more of the major life areas, which are self-care, use of language, learning, mobility, self-direction and capacity for independent living
